The Transitional Housing Program provides short- to medium-term structured housing for people working toward independence after homelessness, instability, mental health challenges, or substance use.
It’s designed for Dane County residents with a primary mental health diagnosis who need daily structure before independent living. Residents get a safe environment, 24/7 support, trauma-informed care, behavioral health services, and help building daily-living skills.
Length of stay varies with your progress and readiness. The goal is to move from instability to independence with real support, not just temporary housing.
Transitional housing isn’t the right fit if you need inpatient hospitalization, detox, or crisis stabilization — we can route you to the right program.


Getting started: complete the online application or contact Tellurian. Applicants must reside in Dane County and complete an assessment with an admissions counselor to determine needs and placement.
Residents receive structured daily routines, clinical and peer support, medication and symptom management, employment assistance, money-management training, daily-living skills, trauma-informed care, and therapy.

Medication-Assisted Treatment (MAT) pairs FDA-approved medications like Suboxone®, Sublocade, Vivitrol, and Brixadi with therapy, relapse prevention, and ongoing clinical support to reduce cravings and lower the risk of return to use.
MAT works best as part of a plan, not on its own. At Tellurian it's integrated with residential, outpatient, and IOP counseling, individual therapy, group support, and recovery planning. We treat opioid use including heroin, fentanyl, and prescription opioids.
