The Community Intervention Team (CIT) provides community-based clinical case management for adults living with severe and persistent mental illness. Using a client-centered, wraparound approach, our team meets people wherever they are — at home, in the community, and throughout daily life — rather than waiting for them to come to us.
CIT brings together psychiatry, nursing, peer support, and a full team of case managers, so residents get coordinated care from people who truly understand their needs and goals.
All referrals are processed through the CCS Portal which can be accessed here.


All referrals are processed through Dane County's Comprehensive Community Services (CCS) portal. Once enrolled, your CIT team completes an assessment and builds a plan around your mental health, stability, housing, and daily-living needs — then delivers that support directly in the community.
Care is ongoing and adjusts as your needs change, with the goal of greater stability, independence, and quality of life.

Medication-Assisted Treatment (MAT) pairs FDA-approved medications like Suboxone®, Sublocade, Vivitrol, and Brixadi with therapy, relapse prevention, and ongoing clinical support to reduce cravings and lower the risk of return to use.
MAT works best as part of a plan, not on its own. At Tellurian it's integrated with residential, outpatient, and IOP counseling, individual therapy, group support, and recovery planning. We treat opioid use including heroin, fentanyl, and prescription opioids.
