The Madison CIT Case Management program provides community based clinical Case Management services specializing in supporting those with severe and persistent mental illness. With a client-centered wraparound services approach CIT is able to effectively meet clients where ever they are at in the community. With psychiatry, nursing, peer support, and a comprehensive team of case managers the CIT team is able to truly meet the needs of those they serve. All referrals are processed through the CCS Portal which can be accessed here.

Diagnoses Primarily Served
Medication-Assisted Treatment (MAT) pairs FDA-approved medications like Suboxone®, Sublocade, Vivitrol, and Brixadi with therapy, relapse prevention, and ongoing clinical support to reduce cravings and lower the risk of return to use.
MAT works best as part of a plan, not on its own. At Tellurian it's integrated with residential, outpatient, and IOP counseling, individual therapy, group support, and recovery planning. We treat opioid use including heroin, fentanyl, and prescription opioids.
